Case Study: Immediate Pain Relief Restoring Shoulder Motion in Severe Arthritis

An 87-year-old male recently came to our practice with severe, chronic left shoulder pain that had taken over his daily life. He rated his pain a 10 out of 10 and had lost so much function he could no longer put on or take off his shirt without significant difficulty. On exam, every attempted shoulder movement was slow, choppy and visibly painful, causing grimacing. His active range of motion was limited to just 40–45 degrees.

Prior imaging showed severe osteoarthritis, extensive degenerative changes, calcification, and bone loss, the kind of structural damage that conventional medicine reserves to pain management alone.

We administered perineural injection therapy to a series of shoulder and arm points. Shortly after, we retested his range of motion. The result: rapid, smooth movement with no grimacing, through a range of 55–60 degrees, and his reported pain dropped from a 10 to a 4 out of 10.

This treatment did not reverse his arthritis or rebuild lost bone. What it did was begin quieting the painful, dysfunctional nerve signaling driving his suffering, a clear example of how nerve-targeted pain treatment can produce fast, measurable results even in advanced joint disease.

How Perineural Injection Therapy (PIT) Relieves Pain

Perineural Injection Therapy targets irritated sensory nerve endings just beneath the skin. Injury, arthritis, overuse, and surgery can cause nerves to fire pain signals long after the original trigger, a process called neurogenic inflammation. A simple dextrose-based injection placed precisely along affected nerves helps calm this abnormal firing, often restoring pain relief and mobility within minutes. PIT is increasingly used for chronic joint pain, nerve entrapment, post-surgical pain, and pain that hasn’t responded to physical therapy, medications, or injections like cortisone.

How Neural Therapy Resets the Autonomic Nervous System
